We are planning to implement SMS gateways provided by Skyline (DMS-MGW-HDW). However, it is not completely clear how many gateways we need.
We are operating a DMS as a failover pair, so usually both DMAs are up and running, but only one is online, the other is offline. Ideally we would have one SMS gateway at a third location with good mobile reception and reachable by IP management network, so whichever DMA is online can work with the gateway. Is this possible? Or do the SMS gateways need to be connected physically to the DMAs, hence we actually need two gateways to empower both DMAs to be able to send SMS when online?

You should only need a single SMS gateway, which can be located at another location even if only a single source IP is allowed.
I assume that your main-failover DMAs use virtual IP addressing (sharing the same IP address). In this case, during the failover from one DMA to another the DMA IP address as seen by the SMS gateway will not change.

My experience is that SMS gateways provided by Skyline are from SMSEagle, and those are IP based modem. (not connected via serial or COM port to an DMA)
This means that online Agent would need to reach the configured IP address, follow the documented example and you should be ok:
